The therapist will identify the needs and barriers to care for the clients and coordinate with Peer Support Services to provide resources and support following a crisis event. They will work closely with Youth and Family, Wraparound, and IIBHT. Primary Responsibilities: * The QMHP is required to connect with clients within 56 days following a crisis event. * Provides in-home, outpatient, and/or community-based treatment services through assessments and evaluations, individual and family treatment which promotes symptom reduction, resiliency, wellness, independence, and safety for individuals within the Youth and Family, Wraparound, and IIBHT programs and supports through prescription of outpatient treatment services that are client-centered, trauma-informed, Evidenced-Based Best-Practices whenever possible; collaborates with clientele and in consultation with team; and completes Safety Plans for those individuals that are at risk. * Whenever possible the QMHP will collaborate with clientele and in consultation with team; completes Safety and Recovery Plans for those individuals that are priority one, those at risk, or for those having a history that indicates that a Safety and Recovery Plan would be of benefit. The clientele is both adult and youth and family. * Maintains and completes required records, reports, and documentation as required by departmental policy and state law; provides documentation and written communications. * Participates in professional development through individual and group supervision, unit and agency staff meetings and workshops. Develops, modifies, and/or obtains training programs and materials in consultation with supervisor and program manager. Promotes peer professional development by sharing training materials and/or training others in successful interventions. * Provides oversight to and clinical direction to post-crisis QMHA's and Peer Support Specialists. * Emergency Response: This employee has been identified as a staff member expected to report for duty following activation of a public health emergency response plan.
